What does private health insurance cost in Spain?
The cost of private health insurance in Spain depends on several factors, including your age when you join, the geographical area of cover, any excess or co-payments and whether the policy is for one person or a family. For a single person in their early 40s, monthly premiums may start at around €90–€150, while a family of four may pay roughly €280–€450 per month. These are indicative ranges only; the final premium is calculated individually.
Most insurers also require a health questionnaire. Pre-existing conditions do not automatically mean that cover is impossible, but they may lead to exclusions, special terms or a different plan. We explain the options clearly before you apply.

Seguridad Social or private health insurance?
Spain has a public healthcare system, the Seguridad Social, alongside private health insurance. Access to the public system depends on each resident’s personal circumstances. For many expats and residents, private insurance also provides faster access to specialists, private hospitals and greater choice.
The right option depends on your residency situation, age and needs. We explain the differences clearly and compare suitable private policies.
Healthcare for expats and residents in Spain
The availability of private hospitals, insurers and professionals who speak different languages varies by region. Major cities and international areas such as the Costa Blanca, Mallorca and the Canary Islands have extensive private medical networks. In more rural locations, it is especially important to check the medical network included with each policy.
Public healthcare
Seguridad Social and public health centres
- Public healthcare is available to people who have the relevant entitlement under the Spanish healthcare system.
- Waiting times for certain specialists can be longer, depending on the region and type of treatment.
Private healthcare
Private doctors and hospital networks
- Private insurance can provide faster access to contracted specialists and private hospitals, depending on the policy network.
- In international areas, some doctors and clinics also offer consultations in English.
Regional access
Specialist care across Spain
- For highly specialised treatment, patients may need to travel to a larger city or another island, depending on where they live.
- When comparing policies, we check that the medical network suits your location and likely healthcare needs.
Frequently asked questions
Do I need health insurance as a resident in Spain?
It depends on your circumstances. Some residents have access to the Spanish public healthcare system, while others need or prefer private health insurance. Travel insurance is not a substitute for full healthcare cover for residents. If you need insurance for a visa or residence procedure, specific policy requirements may also apply.
Seguridad Social or private health insurance – which is better?
The Seguridad Social provides comprehensive public healthcare, while private health insurance can offer faster access to specialists, private hospitals and greater choice. Many residents use both systems as complementary options.
Can I combine public healthcare and private health insurance?
Yes. Many residents use the public healthcare system when they are entitled to it and also take out private insurance for greater choice, shorter waiting times and access to private hospitals.
Up to what age can I take out private health insurance?
The maximum entry age varies by insurer. Some providers accept new applicants at older ages, and specific options may be available for senior clients. We compare several insurers according to your age, health and circumstances.
How are pre-existing conditions treated?
Pre-existing conditions must normally be declared when you apply. Depending on the insurer and the condition, they may be accepted, excluded from cover, subject to special terms or, in some cases, declined.
Why use an insurance broker who advises in your language?
An insurance broker who advises in your language can compare several insurers, explain cover and exclusions clearly and support you after the policy starts. This gives you one point of contact for questions, changes and claims.
